Damstra Mining Services are no longer accepting EFT payments for any transaction prior to invoicing. Alternative payment options are: Purchase Order, cheque (must supply a cheque with paperwork), & credit card. Upon receipt of invoice by your company, EFT payments are accepted.
The process used to book inductions has changed. Please read the process outline detailed in this document.

If you are required to work at the West Wallsend you must have made either a booking for a Generic Induction or attended the Generic Induction prior to submitting all required paperwork to DMS a minimum of 5 full business days prior to the Familiarisation date. If you have confirmed a Generic induction booking you must have attended that Induction prior to the Familiarisation date.
West Wallsend Colliery #2 is located in Killingsworth, on The Broadway. This site is owned and operated by Xstrata Coal.
HOW TO BOOK A WEST WALLSEND SITE FAMILIARISATION
STEP 1: Select a date off the website for when you would like to complete your West Wallsend induction. The inductions are held on every second Monday, for upcoming dates please scroll down.
STEP 2: Download the paperwork from the Paperwork menu.
STEP 3: Complete the Xstrata NSW Generic Registration form, once per person, per generic. Print it out, sign the authorisation then either fax or scan and email.
STEP 4: Complete the Xstrata NSW Site Familiarisation Registration form print it out, sign the authorisation then either fax or scan and email.
STEP 5: Complete a letter of competency on the company letterhead stating:
• How long the employee has worked for the company and what their job title is.
• How many years experience the employee has in the tasks to be performed on site.
• What tasks will they be performed on site.
• State that you find the employee competent the perform the tasks described.
• Ensure the letter of competency is signed.
• If working underground the letter of competency must detail the experience the employee has working underground.
STEP 6: If the company is new to Damstra a Company Details Form needs to be completed in order to register.
STEP 7: Send to Damstra (inductions@damstra.com.au or fax 02 6571 4399) the Xstrata Generic Registration Form, Xstrata Site Familiarisation Registration Form, Letter of competency, Coal Board Medical Less than 3 years old, or equivalent by a doctor, please note that preferred medical providers are Coal Services Health, Ethos Health (previously “Physio at Work”) or Chromis. Medicals from any other providers will need site approval from the Safety and Training Department prior to an induction booking being made, see the Pre-Familiarisation Checklist for exactly what is required. If not previously supplied current proof of ID must be provided and qualifications that support what the employee will be doing on site. If you require the underground induction please follow the below Underground steps before sending off this information to Damstra.
STEP 8: Damstra will check and process your paperwork before sending you a confirmation letter.
STEP 9: The employee attends the site familiarisation conducted by a TESA trainer.
STEP 10: The trainer will then send the assessment to Damstra by 2pm on the day of training, who active the site familiarisation and enable their site access.

HOW TO BOOK A WEST WALLSEND UNDERGROUND INDUCTION IN ADDITION TO THE SITE FAMILIARISATION STEPS.
STEP 1: Select a date off the website for when you would like to complete your West Wallsend induction. The inductions are held on every second Monday, for upcoming dates please scroll down. The underground induction is held on the same day the site familiarisation are held on, therefore it is advisable to do the underground induction directly after your site familiarisation
STEP 2: On the Xstrata NSW Site Familiarisation Form tick West Wallsend in the underground section and provide a date.
STEP 3: Send a copy of your CABA card along with the rest of the site familiarisation requirements.
STEP 4: Damstra will check and process your paperwork before sending you a confirmation letter.
STEP 5: The employee attends the site familiarisation conducted by a TESA trainer.
STEP 6: The trainer will then send the assessment to Damstra by 2pm on the day of training, who active the site familiarisation and enable their site access.
